In your HaloPC folder, there's a file called Msxmlenu. Run that file and click "Repair"

No.

I don't know much about Halo CE (custom edition), but have you downloaded the User Interface replacement for the maps?

You can go here to do so: http://hce.halomaps.org/index.cfm?pg=6&sid=26

Also there is an explanation on the top of the above page: Posted by: hce.halomaps.org

The files in this section are used to replace the user interface of the Halo CE game. The user interface file controls the animation behind the menu system when you start the game. To use these files you must first backup the UI.MAP file - place it in a holding folder - then replace the UI.MAP file with one of the ones in this section. The UI.MAP file is located in the C:\Program Files\Microsoft Games\Halo Custom Edition\maps directory. Remember to backup your original UI.MAP file BEFORE replacing it.

That's about all I can help you with. Sorry.

Cheers.

download a UI (User Interface) and replace it with the one you have in your Custom Edition's "map" folder. from there download each of the CMT levels.
also note: the UI must support the campaign feature.
