Ok, since custom games are done where all users connect to the host party's xbox and play the game from there, I would start out to describe this strange happening as a possible series of mods. First, we can thank XBL for being a tracker for us, so that we do not have to input IP addresses or anything in order to connect to a custom game. I'm sure its possible to mod an xbox so that it would scan for IP addresses hosting custom games, and then a second mod in order to bypass the "closed" game status. Third, a modder would then need to modify the print files that print out on-screen the things that are happening in the game (kills, deaths, suicides, etc), so that it would not show game activity, like joining and leaving the game, as well as kills. Finally, a modder would need to cloak it's name/gamertag/ID from the player list.
All this to say that these ghosts put a lot of effort into making sure that they're ghosts, but I still believe that these are probably modders "hacking" the custom games, and not some epic phenomenon of Halo 2.
