Artificial intelligence is no match for natural stupidity
Similar thing happened to me. Right before starting the Gravemind level, I got the "insert your original Halo CD" message and it wouldn't play my saved game. Bummer. I played the game in the Xbox and believe me, the Arbiter does not end there and once you get to finish the game (if you haven;t done so in the Xbox already) then get ready for some trollin' in the forums.
The missing map connundrum It is easy to solve though. If you try re-load your saved game, it will tell you that the data doesn't match live, so you can either continue without achievements or restart the level. DO NOT SELECT EITHER.
Cancel everything (I had to Ctrl-Alt-Del) and re-launch the game, This time choose to start the game by selecting the level. It will prompt you that your saved game will be lost. That's OK. In the list of available levels, you will see the Gravemind level available. Select it and the story/game will continue. You can check my gamertag (Waldo15) so you see I indeed went past the Gravemind.
I can list now how many borked things are in the game. It is not that is unplayable... is just that the Experience is far from painless...
