OK, here's how you would do this. This is what my friend and I did when we had exactly the same problem.
I don't know if you're using an original xbox or a 360, but it should technically work the same way. First, both of you need to manually configure your xboxs IP addresses to a set one, probably 192.168.1.XX and set it to one like 20 or something, so that you will never have that many hooked to your router at once, so no overlapping later. Set your subnet to 255.255.255.0, and then your default gateway should be 192.168.1.1, and if that doesn't work, put a 0 at the end instead of a 1.
Then, go into your router, find the setting that contains something about DMZ. Enable the DMZ and set that as the same IP as what you configured your xbox to. This forwards all ports to your xbox, but still acts as a firewall to all other systems on your network. And since there's really not much fear of it being hacked, since there's no point, it will be safe.
DMZ forwards all ports to your xbox, enabling you to get a connection to your friend easier. If it doesn't work, then I've got no more suggestions.
Disclaimer: If you have a modded box for any reason, then don't do this, because if you're running some other program on your xbox and you open all ports, it could get bad. I'm not sure what it might do, since I don't deal with modded boxes, but I work with computers, and so that should help. Good luck.
