I have a theories about the system's formula for the ranking system.

Number of member's Post; P
Number of member's ban; B
Number of member's warn; W
Member title;
Heroic; P/1250 x((15%xB)+(5%xW));
Legendary; P/2500 x((20%xB)+(10%xW));
Mythic; p/5000 x((30%xB)+(20%xW));
If the user reaches to 1 in either one of those formula, it means that the user has been promoted to that certain title. I still need to figure it out about the preffix system, but that will take awhile before I come up with a formula of some sort.

Yes, I have a problem. Your theory doesn't comprehend the fact activity requires consistency, not accumulation. Also, you tried to combine all the systems into one, that couldn't be right.

-Activity levels
-Age of account
View them as separate.
The trust points, activity levels and the age of the user account affect the prefix of the title, not the member title.
You see, the more active you are, the more likely you get a prefix, but not that soon. Same for veteran, they have been here for almost five or so years, and they gain a veteran prefix, because of the age of the user account. The member title affects base on how your post, ban, and warnings. I have been through this and contacted with another user in here who has been banned before, told me that one ban could drop the user member title.
