This is concerning the Exalted prefix:
I have received the Exalted prefix on, or slightly before, new year's day of 2010. I have been a Bungie.net member since four years today. I know I was banned, and possibly warned, back in May or June 2007.
Now, I believe the Exalted prefix requires at least the three active years as a Bungie.net membership and no bans/warnings for around three years. I know the membership prefixes and titles are still up to great debate, especially the ban-related prefixes, so here's my input I have been thinking about since receiving the Exalted prefix. The 1/2/3 year rules for the ban related prefixes only seem to apply to members who have never been banned before (ie those who have a perfectly clean slate). For users who have been banned before, a few more factors seem to get thrown into the mix. For example, the duration of the ban received looks to be one of the more influential factors, as users who have received shorter bans seem to take less time in obtaining a ban related prefix than those who have received a longer ban.
Also, something that I think a lot of us have noticed, the New Year had a huge impact on many user's prefixes, mainly the ban related prefixes. Just by the looks of things, I would say that the New Year acted as a small "slate cleaner" of sorts to those who received small bans in the past years, allowing many members to qualify for a ban related prefix on that day. Since your last ban was a while ago, it's understandable that you qualified for Exalted once your slate was slightly cleaned. ^_^
I think you may be right with the tabula rasa effect of sorts. I've received one single ban in my BNet career, and it was about 26 months ago if I had to estimate. Sometime around November of 2007. I think it was a 2 week ban, and it happened in the old Files forum when that was still running. Anyway, since then, never even sniffed trouble really. I think I bumped up to Exalted either on Christmas or New Years (my BNet anniversary is on Christmas day).
