Back in the days I did change about a bit more, the beginning of the month certainly was when it generally happened.
Now, unless I drop in activity, or get banned, I'm going nowhere until 2013 and Exalted :S

I got Legendary for the first time yesterday of last year, and after a warning in June, regained it on the first day of September. So I'm fairly certain of this also.

I'm almost 100% certain that the first of the month affects title changes. Case-in-point: me. I was banned and on the first of the month I went from a Member title bar to Mythic.

How active do I have to be to atleast go to a Heroic?

I have the Noble prefix title:
Noble - Active member for 2 years, with no bans for at least 1 year. If banned/warned, you may or may not keep the title, depending on length/number of bans
But according to this, I should atleast be a Heroic by now:
Heroic Member
* title bar color is gray
** active member for about 6-12 months

Ugh, I guess it doesn't really matter, it's just a title.

Start posting more frequently, and eventually you will gain Heroic.

You need to be decently active. By posting around 7-15 times a day constantly and of course, make sure the replies are not just spam.

All you need to do is post in the Public Forums more. You 300th post was on the 7/22/2010 and your PPD is 1.53, thats not enough to be Heroic.

More active, try 7-10 posts a day. Your post history goes back to July, which isn't very active.

Yeah, it seems as if titles get handed out on the 1st of each month. I'm sure they get handed out on other days, but it looks as if quite a few people ranked up yesterday. :)

Still on the long crawl to infinity that is mythic. Also awaiting noble (around 5 months away) or intrepid (who knows?) title.

I'm pretty sure it's impossible to lose the Intrepid prefix from getting banned. Last time I checked the only for sure way to lose your Intrepid prefix is to either wait until the 3-year mark from your last ban; or you would have to reach the Fabled prefix.

Well, unfortunately, you can't really get rid of that prefix easily, as I'm sure you know. Either you'll have to get yourself banned for at least a week or two every few months, or you can just stop complaining and live with the prefix.

I think it has to do with your account age, wich is why you got Heroic with very few posts.
