The "active for 2-3 years" thing isn't a exact description, it's a good estimate for average members of the time Duardo posted.
What I'm basing my answer off of : (The part you can skip.)
As you probably already know, base titles aren't given for time spent, ban amount, activity and so on but with some crazy equation (that no member or moderator knows and is deliberately made to be confusing and difficult to figure out) that uses data points to figure out your what we call "trust points".
Trust points aren't where this road ends, you then compare all the given "trust" point data from every user and fix it all into a equation that makes it so only a certain (set) percent amount (in a notable theory that I like to take for granted) of members get mythic, legendary and heroic.
To answer your question with the given information, I can theorize that people with the exalted prefix and a lower base title could have a lower base title due to:
1: How the base titles work nowadays is different due to the larger amount of older members with a already very high trust rating.
2: The user might indeed not be as active as is required for the title.
3: The user might of in fact gotten several warnings or bans when their account was much younger.
4: Other: You could never know for sure, really.
If you're wondering why I'm exalted (a proud user with no bans or warnings), I would like to theorize that it's because of how immature I was when my account was younger and how I didn't really visit the B.net from time to time.

You know, I've been wondering, and for some other people as well. So exalted means that you have had no bans or warnings for 3 years, and to get Mythic you have to have been active for 2-3 years, so how come a lot of people with exalted don't have mythic? Is it just because they aren't active enough or weren't active enough at certain points?
