There is no set way to make a custom avatar. It depends on how much time/effort you wish to put in, and how much you know about computers, programs, and how they're used. Most common program used is Photoshop by Adobe. Unfortunately, it's very expensive, around $700. There are other alteratives, but I don't know them. Try searching google.com for image editing. Then you take an image or images that are bungie related, and use said program to edit it and make it your own. You can add text, graphical effects, removes parts of images, change the translucence. It can be challenging, but you can find tutorials all over the web regarding editing images, so just be patient, and look for what you need. And you'll eventually get the hang of it.

Yes, but you can submit them for public use.
