In the Halo guide, this is listed as "glitch #244." Happens all the time and is the most noticable when you throw a nade and then die and then during the rejoin countdown listen for your nade to explode but it never does.
The reason it doesn't explode is because you really NEVER threw the granade to begin with. If you watch closely, it will happen when you are alive too. Throw two granades as fast as you can. You will notice that the first nade is thrown and then your arm throws the second one and you hear the nade bouncing, but the nade was never thrown and never blows up.
When you die, it LOOKS as if you threw a granade and, in fact, you may even hear it bouncing, but in reality, you died before the nade was thrown and, therefore, the nade REALLY wasn't thrown.
"Glitch #245" is similar. This is when you kill someone and it appears as if a nade suddenly is "live" from the dead person and it explodes. They are often referred to as "lag nades."
"Glitch #248" is also similar. This is when you kill a person and they "appear" to be able to still shoot you for a few seconds after they are dead. This is most apparent with rockets or fuel rods.
Anyway, the answer to your question is that it is one of the hundreds of glitches in the game.
