Occasionally when I am playing, I will get the message "we had to sign you out of live because someone else is signed in with the same account." Is this an indication that the account has been compromised? Is there some way to fix this?

This is a very bad sign. Most likely someone has access to your account. This means someone has recovered your account by knowing your e-mail and knowing your password. Did you hand out your password/e-mail to anybody?

If somebody has access to your account it might be wise to change the password as soon as you can. ~Cm

^^^ That and what SPB said. If you do try to change your password and it won't let you, you need to call MS and delete the account and start over. It sounds like you've been had, maybe by a friend or an acquaintance.
