1. Reach isn't incredibly far from Earth. It wouldn't make sense for it to be, considering Reach is the military hub of the entire UNSC, for the most part.
2. Sierra is not, never has been, and likely never will be a callsign that is used for only Spartans. It is the military code sign for the letter S. Thus far, it has been used for Spartan callsigns as well as a Pelican in the novels, if I'm not mistaken.
3. 30+ Spartans-IIs were either killed or crippled in the bioaugmentation procedures in 2525 when they were only 14 years-old.
4. The second training of Spartan-IIs has never been confirmed to even occur, so we can not say what fate has befallen a theoretical group of people. There was a subsequent Spartan project in which Spartan-IIIs were created, however. There are a small handful alive and well at the end of Ghosts of Onyx, and 315 others whose fates have not yet been revealed to us.
5. The events that occur in "The Fall of Reach" occur and end directly before Halo: Combat Evolved. "First Strike", which also has a large plot story about Reach actually occurs after Halo: Combat Evolved.
6. Master Chief is not headed for Reach. The planetoid at the end of the Legendary Ending is unidentified, and is likely of Forerunner nature given its incredibly large Forerunner symbol that can be seen as the light emanates from the background. Reach and this planetoid reside in different locations with different suns and the surroundings differ greatly as well.
As for the rest of the Covenant, it is implied months later that the Covenant had crumpled apart and was no longer a threat.
