Originally, it was called SLASO (Solo Legendary All Skulls On), but we didn't like the name. It felt awkward. It was also possible to "cheat" by saving and quitting to circumvent Iron sending you back to the start of the level when you died, and if you did it co-op, then not only did the name not work, but Iron only sent you back to the last checkpoint.
Thus, with a few rules added to SLASO, "Mythic" was born.
Da rulez:
for solo:
-Legendary
-All skulls on
-No saves (missions must be played start to finish in one sitting, though nothing prohibits pausing the game while you do something else)
-Scoring turned on (this way Bungie.net tracks it and you can prove you beat it with the saved film + gamestats)
for co-op, it's the same deal except every time ONE of you dies, the party leader must manually restart the mission. It's become standard procedure for both solo and co-op runs to quit the game and restart from the beginning so that only a single, successful run is in the film.
Also, not meaning offense, but in the future use the "edit" button to edit your post if nobody has posted after you. Multi-posts are inefficient and undesirable. They may also be against forum rules, I'm not sure.

To further add to the explanation, the term "Mythic" didn't just pop out of nowhere. Bungie's standard ranking systems goes Normal, Heroic, Legendary, and Mythic. But there is no "in-game" Mythic difficulty in the Halo franchise. Likewise, the hardest possible in-game setting is Legendary with all skulls. So naturally, the hardest difficulty should be given the highest title. Thus, Mythic was born.
