Clean the whole surface of the disk with soap.
Check to see how much usable memory you have left in the 360 HDD. If it's close to filling up then delete non-essential saves or downloads or whole games.
I have a laser cleaning disk. It has a little brush on it and cleans off any dust on your DVD laser.
I think you can still clear the HDD cache memory. Halo 2 likes to load a lot of the game to your HDD. Maybe when you first popped in H2 it did not load properly and it's in the cache memory.
I'm just guessing here and my last statement is a wild guess.

In answer to your suggestions, I've never heard of cleaning your disc with soap?? but I have tried various other cleaning methods and the disc doesn't seem to be dirty or damaged(nothing serious anyway). I still have 128GB left so I don't think it has anything to do with space. I don't think its the DVD laser since the Xbox (Slim) is fairly new (Halo 2 wasn't working on my old 360 either) and I've cleared the cache memory several times now to no effect.
One day the campaign was working, the next day it wasn't. I'm not sure if this means anything but I haven't been able to load the campaign since the day Halo 2's online ended, I played my last match online and the next day nothing was loading (I thought that not being able to sign in anymore had something to do with it).
