Concussion Rifle shoots explosiv Plasma Rounds, not Grenades.

It seems like the plasma launcher fires plasma grenade like projectiles. They also fall over time indicating that they have some mass that can be affected by gravity, unlike directed energy weapons.
The Wraith's Plasma Projectile arcs.

The official information about it also states that the Plasma Launcher does, indeed, shoot Plasma Bolts.

It shoots plasma grenades, hence the name. They can be called bolts just like how arrows for a crossbow are called bolts.

As for the wraith, it is likely very dense plasma, explaining the arc.

It's name is the Plasma Launcher, not Plasma Grenade Launcher. If it did, indeed, shoot grenades, how come it doesn't reload, like the Fuel Rod or Brute Shot? How come it has a battery?

The Plasma Rifle, Pistol, Repeater, etc. shoots Plasma Bolts. It is essentially the name given to globules of Plasma.

Going back to it's probable Forerunner roots; these weapons could work on an extra-planar level. The crystals could be attracked to a target's shadow on an alternate plane, and the weapons firing mechanisms could also communicate that way.

If you read Dr. Halsey's journal given out with the limited edition of Reach, it explains that the workings of the Covenant needler is completely unknown. The internals do not make any sense. It is as if the user pulls the trigger and somehow the projectiles 'magically' fire. There are no physical, electrical or radiative connections between the trigger and the firing mechanism.

I suspect that Bungie just couldn't be bothered explaining it.

You stick the needler big crystals in, as seen in Halo CE and...

Stuff happens.
