I'm going to keep this brief as that's quite a list.
1)Yes, Halo Wars is considered canon. Red Team at the time (2531) consisted of 3 Spartans. They were picked up at Arcadia, and when they were confirmed MIA along with the Spirit of Fire. Red team command was then transferred to Fred.
The Spirit of Fire crew along with Red team found the flood on a Shield World infested with them. They were presumed MIA after the conclusion of Halo Wars, so it's safe to assume that they had not made it back by the events of the Halo Trilogy. However if they had made it back, then the details were kept secret.
2)The Artifact that was found within Reach that altered time/space was known as a crystal. It's abilities had allowed vessels to slipspace travel further also using less energy. If they approach close enough to the crystal it bends time/space.

The issue with this was that the Covenant could track down where-ever the crystal went due to the emissions it left. Thus it had to be destroyed. The remaining shards left behind were given to Truth by Tartarus.

They had however transported back in time by finding the crystal which leads to some confusing events, if anyone can clarify this part, that's great. I don't have my first Strike novel on me anymore.
3)The Data pads are hidden computer pads, the pads contain their own, self-contained story, presented as exchanges between members of a mysterious AI collective known as the Assembly.*
The Assembly was a legislative group of Human artificial intelligences active from 2310 to 2552. The minutes of their meetings were recorded in the Data pads in Halo: Reach*

4)He calls her ma'am. That's the female equivalent of Sir. His accent often leads to this misunderstanding. Put the subtitles on and this would be seen as true.
5)Aftermath
When Installation 05 was last seen, the Flood were seen having complete control over High Charity and several of the surrounding fleet. The Great Schism, a civil war between Covenant species, split the empire into Loyalist and Separatist factions. At the end of the battle, what is left of the Sangheili fleet sterilized the installation, most likely by means of glassing the surface.
The hologram in the Citadel on Installation 00 shows Installation 05's surface as completely barren.*
