Because in Reach, all the Spartans in Noble didn't have them. Forge didn't have one. Terrance is the only one that does have one in the games.

Considering the role they play (FOF) I'm surprised they are not on every soldier you see.

A design oversight or non-canon now?

I believe the neural interfaces for the main part simply aren't visible. There are some that are, but Hood's command one is probably far more advanced then anybody else.

Also, Keyes had a neural interface, the back of his head/neck didn't have the plug. Supporting my view.

A good amount of times actually. Though naturally it only extended to Carter, Jun, Kat, and Jorge.

In any case as Cmdr stated they don't need to be visible on the neckline to exist in the noggin. Keyes being the prime example since his implants were swimming around the insides of his liquified brain despite the lack of markings behind his head when he was still alive..
