Well, you either have to be the server admin or have rcon enabled. My guess is that you're the admin. You need to first have the console enabled. To see about how to enable and access the console in Halo, click the link in my sig '';aM;'' Inspire's Ultimate Guide to Understanding Rcon. It'll explain in detail how to enable the console, and it also goes into great depth about how to use rcon if you're not the server admin.

Anywho, once you get the console enabled, you type

sv_ban [playername]

into the console. Now, some players use all kinds of crazy alt-codes in their names, so it might not be practical to use the name of a player to ban him. Halo assigns each player a number (this number can change between games) that you can use in place of the player's name. to find out the player numbers, type

sv_playerlist

into the console.
