Are ultra bans ip bans?
I think that would be overkill. An IP ban restricts you from logging on via specific IPs.

An ultra ban sounds simply like it won't let you do anything upon signing into your ultra-banned account. Similar in how if you sign into an account that's been banned, you'll simply be unable to post to the main forums, except now it applies to everywhere you can interact.

As for "who's ever had one of these?", I could take a guess at a couple impure candidates, but I don't know nearly enough about the underworld of Bnet to know for certain.

No. It blocks you from accessing any part of the site (including the homepage) while signed into an account that is "ultra banned".
