1. Know your basics. Know where you intend to take your clan, know how you intend to do so, and know who exactly your target is. You need to know if you're going to be a loose confederation of for-fun players that will refer to themselves as a clan, or if you're going to be a fully-fledged, competitive clan. Let's say you decide to be competitive. You then need to know how you're going to get there (selective recruitment? recommendations? approval only? holding lots of practices?).
2. Decide how to organize yourselves. Who's going to be leader? How will leaders be appointed? From what I've seen, the best clans are formed when led by a group of about 4 or so co-leaders, or when the title of "leader" is taken out completely and the clan is made "for the clan" in the self-serving sense. Also, try to set up a meeting spot, be it a bungie.net group, an actual seperate clan board, etc, etc.
I will guarantee you that if you don't have the mentioned in mind, you WILL fall apart as a clan. Good luck, and hopefully I'll see you on the battlefield someday...
