Many generals feared X-day. Otherwise known as the Invasion of Japan. Assuming the nuclear weapons did not force Japan to surrender, the only option left would have been the full scale invasion of the Japanese home islands.
The reason this was so feared was the resistance the Japanese put up on Iwo Jima and Okinawa. Although the Japanese fleet was all but destroyed, they still had a 4 million man army ready to defend their home. Not to mention the millions of civilians that were taught the Americans will -blam!- and kill every civilian, so they were openly hostile to the American forces.
Looking at the casualties from the Iwo Jima and Okinawa, the casualties on both sides would have been in the millions on this scale, and the war could have easily gone on until 1947. So, how do you think the coarse of history would have changed had we invaded the Japanese home islands?
That is impossible to say but i would think we would have been victorious at an amazing cost as you have stated.
The atom bomb not being used may have made the cold war a bit less dangerous but i cant think of anything else.
