I think you're conflating two things typically found on a job application: work history, and references.
Depending on the application, they could be one and the same. I'd probably call my old boss and see if it's okay to list him as a reference (even if it's just under work history). Nowadays, a lot of managers fact-check what applicants put as their work-history to make sure they aren't just making it up. Maybe it's just me, but I see it differently. I've seen work history sections that ask for contact info for past supervisors, and I'll answer that without asking the individual, because to me, that is not a reference in the traditional sense -- it is a matter of fact that that person was my supervisor, whether they want it to be so or not, so I see little reason to ask them about it.
